An application fee of $250 before March 1st or $300 after March 1st (per family) to be submitted with application.

A copy of the most recent achievement test scores if you have them.
Grade report cards of the most recent quarter and one from the previous if applicable.
A copy of any divorce/custody decision as it pertains to the student(s).
A signed medical release form for each student.
A signed liability waiver for each student.
A signed statement of faith.
Please be sure the pastoral reference form has been sent to your pastor.
